PROGRAM INTRODUCTION

The CALTCM Summit for Excellence is designed to engage and benefit direct care practitioners - all members of the post-acute and long-term care (PALTC) interdisciplinary team and administrative leadership. Engaging content is available for the entire interdisciplinary team and in-person meetings will allow a dynamic and experiential learning experience. Varying clinical topics and three “In the Trenches” sessions give you the opportunity to personalize your educational experience and interact with program faculty and other participants.

This year's annual meeting is focused on person-centered care across the pillars of well-being, offering a diverse range of topics, including:

  1. Physical Wellness: Parkinson's Disease, Osteopathic manipulative medicine, and the future of medicine and technology advancements.
  2. Mental Wellness: Substance abuse, alcohol, and opioid use in LTC, antipsychotic justification, and strategies for promoting mental well-being.

TARGET AUDIENCE

Physicians, advanced practice nurses, gero-psychiatrists, pharmacists, licensed nurses, administrators, and other members of the interdisciplinary team who care for patients and residents in PALTC.


LEARNING OBJECTIVES
By participating in the annual meeting, participants should be able to:

  1. Define concepts, methods, and best practices to incorporate and/or improve person-centered care in the post-acute and long-term care (PALTC) setting;
  2. Advance administrative and care practices to deinstitutionalize the LTC environment and improve emotional, social, spiritual, and environmental well-being for residents.
  3. Identify treatment updates related to common medical diagnoses and medications prescribed in PALTC communities;
  4. Employ best care practices in supporting the mental wellbeing of LTC residents and address substance use and mental health conditions;
  5. Characterize trends in medicolegal and regulatory actions, and implement practices to improve quality of care while appropriately recognizing and managing current and future risks;
  6. Describe policy and regulatory updates and common clinical and regulatory issues faced by PALTC practitioners.
