Individual Annual Memberships

Note: Multi-year options available within registration form.

• Physicians:  $150
• Clinical Affiliates:  $125
• Retired Physicians:  $100
• Retired Clinical Affiliates:  $75
• Students (all disciplines): Eligible for Complimentary Membership - Standard rate is $100

Lifetime Membership

A Lifetime Membership is available to all disciplines at a cost of $1,500.00, please call the Executive Office for processing.

Facility Membership

A special facility rate of $300 is available for nursing home teams - typically a Medical Director and two facility personnel (Administrator and DON). Additional members can be added at $75 per individual. To expedite the registration process, please have the first and last names, email addresses, credentials, and titles for every person you are including in your facility membership.  

Corporate Memberships

Corporate memberships are available.  If you would like to create a corporate membership for multiple facilities please contact Barbara Hulz at (888) 332-3299 for guidance.
