Working Together to Improve Care, Communication and Continuity for our Patients The California Association of Long Term Care Medicine (CALTCM) is dedicated to improving quality patient care in California. CALTCM SNF 2.0® a unique opportunity for nursing homes interested in reducing avoidable readmissions. The CALTCM SNF 2.0® program goes beyond the typical training program and provides custom in-depth, on-site mentorship to participating nursing homes for approximately 6 months. Click Here for SNF 2.0® InformationLeadership and Management in Geriatrics (LMG) courseAugust 2024The 22nd annual Leadership & Management in Geriatrics course will afford participants the opportunity to spend time in an intensive and highly interactive setting with leaders in the field of geriatric medicine and management followed by an exclusive two-month post-course mentorship with program faculty. Participants are encouraged to think creatively and consider novel approaches to creating and managing geriatric practices, and services. CME, CMD, ABIM MOC (for eligible participants), and CE units are available, and additional credits are awarded to those who complete a post-course mentorship project. |
