CDPH Award Announcement: CALTCM LMG-IP Strike Team Project

CALTCM is proud to announce that it has received an award from CDPH's Healthcare-Associated Infections (HAI) Program. Our project, the Leadership and Management in Geriatrics Infection Prevention and Control Leadership Program (LMG-IPC Strike Team) combines the methodology from two established CALTCM programs, CALTCM SNF 2.0® and CALTCM’s Leadership and Management in Geriatrics course (LMG)®.


The program combines leadership training, Quality Assurance/Process Improvement (QAPI) exercises, and interactive team-based training to build cohesive leadership teams that will work collaboratively and effectively to improve processes critical to the delivery of safe, high-quality, person-centered care. Through its intensive curriculum and a series of interactive webinars, this program will focus on emergency preparedness and infection prevention and control.


Nursing home leadership teams will improve individual and team skills to establish better communication pathways focused on person-centered care to enhance resident quality of life and health outcomes.


Eligible nursing homes must meet the following criteria:

 

  • CA nursing home, located outside of Los Angeles County.
  • Rated as 1 or 2 Star Facility on CMS Nursing Home Compare website

Facility leadership teams are expected to include the Medical Director, Director of Nursing, Administrator, Infection Preventionist, and Director of Staff Development. Nursing homes will receive a certificate of completion at the end of the program, and physicians, nurses, & administrators will be eligible for continuing education hours.
