CALTCM Ventures Deep into the Heart of Texas |

Almost 100 Californians were registered at the March 7-11 AMDA annual meeting in San Antonio, entitled “A Mission from the Heart,” and our CALTCM members as usual distinguished themselves among the crowd of over 1600 attendees from the U.S., Canada and overseas. Many Californians presented lectures, including our own Rebecca Ferrini (AMDA’s Medical Director of the Year for 2009) and Rob Gibson, both from Edgemoor in Santee. I had the opportunity to address the Saturday general session on behalf of AMDA’s Public Policy Committee, on some of the top LTC policy issues over the past year. I did not wet my pants in front of the huge crowd, thankfully.

Can This Resident Make Decisions? |

Mr. Doe has been in your facility for 3 months, but recently suffered a minor stroke. He is a long-term homeless alcoholic with no known family or friends, and he has also just been diagnosed with diabetes and you need informed consent for treatment. He states that he is fine, doesn't think he has diabetes, and that he wants to go out to get a drink. He says he'll be back before dark.

The Meaningful POLST Conversation |

As I walk into the SNF on Monday morning, 3 new patients were admitted over the weekend. I flip through the charts and find each patient has a POLST form with some sections completed along with a request for my signature. I wonder:
- Who had these conversations with the patient and family (If not documented on the back side)? Was it an informed, collaborative discussion?
- Were the decisions unequivocal?
- Does the patient have decision-making capacity (if they signed the form)?
- Did the surrogate make the decisions even though the patient has decision-making-capacity?

I have been in private practice since 1996 and I am an Assistant Clinical Professor at the UCLA School of Medicine. As a multi-boarded physician, [board certified medical specialist in internal, pulmonary, sleep and geriatric medicine] I have an opportunity to see thousands of patients in the outpatient, inpatient, and nursing home setting. I continue to remain passionate about patient care and stand by giving my patients the care and time they deserve.
