CALTCM continues to partner with you and your team to bring up-to-date strategies for addressing common and challenging problems in Post-Acute and Long-Term Care.

This year, we are pleased to collaborate with the Northern CA Gerontological Advanced Practice Nurses Association (NorCal GAPNA) to bring you a series of “best practices” that are designed to help you achieve better survey readiness by improving the quality of dementia care of older adults in your nursing home as well as strategizing patient-centered discharge planning and readmission reduction.

Scheduled for a full day November 3rd, we are focusing on practical solutions to reduce readmissions, with sessions on strategies for safe discharges and a presentation of innovative solutions in CALTCM SNF 2.0® by our own geriatrician and lead investigator Dr. Albert Lam. We also focus on effective Dementia care interventions, with a useful workshop to help reduce and manage agitation and aggression in dementia patients as well as a presentation of the impact of CAHF Music and Memory Project in Dementia care, presented by lead investigator Dr. Deb Bakerjian.

And as always, CALTCM mentors are on-site to guide you through our essential hands-on QAPI planning sessions, for a deep dive into all these strategies and how you can apply them to your own setting. The day is rounded up with our popular regulatory landscape updates.
