CALTCM Best Practice Summit  

Pre-Conference Overview

This pre-conference serves as a platform for sharing and disseminating best practices in long-term care.  Implementing the five best practices with proven results will help spread innovation throughout the long-term care community.  Experienced coaches will work with the attendees to develop action plans supplemented with structured coaching to foster sustainable adoption of these best practices.

Pre-Conference Learning Objectives

At the completion of this training participants will be able to:

  1. Describe best practices on managing chronically ill patients;
  2. Articulate objectives and identify metrics to measure an implementation plan;
  3. Identify barriers for implementation of best practices;
  4. Develop a contingency plan to overcome barriers for implementation. 

Pre-Conference Fee

Thanks to the generosity and support from SCAN Health Plan, the $125 Pre-Conference fee is REDUCED to $50 for Members and $75 for Non-Members. Seating is limited for this event, early registration is recommended.

California Association of Long Term Care Medicine designates this Live activity for a maximum of 3.75 AMA PRA Category 1 Credit(s)™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity. This credit may also be applied to the CMA Certification in Continuing Medical Education.

This course meets multiple requirements of the California Business and professions Codes 2190–2196.5 for physician CME, including cultural competency and geriatric credits.
