Are You Finishing Your Notes on the Same Day?

Ashkan Javaheri, MD, CMD PhotoTimely documentation is an expectation across healthcare. Many medical groups hold physicians and advanced practice providers to 24–48 hour documentation windows, often tied to billing timeliness and compliance requirements. In a traditional clinic, that’s achievable — labs, imaging, medications, and prior notes usually live in one EHR.

SNF practice is different.

A clinician rounding at three facilities in a day may touch four different systems — each facility’s own EHR, plus the group’s documentation platform, and sometimes an enterprise system like Epic layered on top. Every login and interface switch adds friction that a clinic-based standard doesn’t account for.

What Should the Standard Be?

I believe same-day documentation should be encouraged whenever possible, with 48 hours as a reasonable expectation for routine notes. Urgent changes in condition, new orders, transfers, and anything affecting immediate patient care still need to be documented and communicated promptly — no exceptions there.

Technology can narrow the gap. Ambient scribes are already cutting documentation time for some SNF clinicians, and post-acute platforms like PointClickCare and MatrixCare streamline charting where medical groups can integrate with them.

Timely documentation matters for patient safety, communication, compliance, and continuity of care. But the standard we hold clinicians to should reflect the realities of post-acute practice, not just clinic-based norms.

So the question isn’t only “Are you finishing your notes on time?”

It’s also: are we giving clinicians the systems and tools they need to do so?
