CMD Summary for California Medical Directors

The recent release of CDPH AFL 26-12 served as an important reminder to skilled nursing facilities and medical directors across California that the deadline to become a Certified Medical Director is approaching. For many physicians, the reminder sparked questions about timelines, eligibility, and the certification process.

For physicians dedicated to caring for older adults, this requirement is more than regulatory compliance. Certified Medical Director training strengthens leadership skills, expands understanding of regulatory and operational oversight, and reinforces best practices that directly impact resident care and facility quality.

As a summary, under California Health & Safety Code §1261.4 (Assembly Bill 749) a skilled nursing facility may not contract with a medical director who is not certified—or who will not become certified within five years of their initial hire date. Medical directors already serving in their role as of January 1, 2022 have until January 1, 2027 to complete certification requirements.

Becoming a Certified Medical Director is education based – there is no exam. Here is a summary of the journey to become a CMD:

  1. Physicians should review the eligibility requirements and determine if they meet them.
  2. Sign up for the Core Curriculum through PALTmed
    1. Part 1: Asynchronous – 100% on-demand
    1. Part 2: Live (Virtual and On-demand options are available)
  3. Earn the additional required CME and CMD hours needed for certification
    • The number of required hours, in addition to the core curriculum, will vary based on board certification and/or geriatric fellowships).
  4. CALTCM educational programs offer physicians the opportunity to earn CME and CMD credits simultaneously.
  5. Medical Directors must log into the CMD Certification Portal to complete the CMD eligibility checklist and application.
