CALTCM Best Practice Summit  

Pre-Conference Overview

This pre-conference serves as a platform for sharing and disseminating best practices in long-term care.  Implementing the five best practices with proven results will help spread innovation throughout the long-term care community.  Experienced coaches will work with the attendees to develop action plans supplemented with structured coaching to foster sustainable adoption of these best practices.

Pre-Conference Learning Objectives

At the completion of this training participants will be able to:

  1. Describe best practices on managing chronically ill patients;
  2. Articulate objectives and identify metrics to measure an implementation plan;
  3. Identify barriers for implementation of best practices;
  4. Develop a contingency plan to overcome barriers for implementation. 

Pre-Conference Fee

Thanks to the generosity and support from SCAN Health Plan, the $125 Pre-Conference fee is REDUCED to $50 for Members and $75 for Non-Members. Seating is limited for this event, early registration is recommended.

California Association of Long Term Care Medicine designates this Live activity for a maximum of 3.75 AMA PRA Category 1 Credit(s)™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
