Annual Physician Membership $150.00

Annual Clinical Affiliate Membership $125.00

Annual Retired Physician or Physician-In-Training $100

Lifetime Membership $1500.00 (Please call executive office for Lifetime membership.)

New Facility Membership
A special facility rate of $300 is available for nursing home teams - typically a Medical Director and two facility personnel. Additional members can be added at $75 per individual. If you wish to sign up for a facility membership, simply click here.
