The CALTCM Quality & Safety Committee is charged with promoting Best Practices in Patient Safety by offering quality educational opportunities to help every California nursing home develop a culture of safety and learn innovative and effective techniques to address quality concerns.  The HSAG Reducing Readmissions Preparation Program offers an ideal platform to meet our objectives and help you track and document your efforts in your QAPI program.

Health Services Advisory Group (HSAG) working with CALTCM’s Quality & Safety Committee, has launched a new program to help nursing homes prepare for the CMS Value-Based Purchasing (SNF VBP) readmission penalties, which will go into effect October 1, 2018, although the data are already being gathered for benchmarking. Join this nine-month journey (January – September 2018) to implement prevention strategies that should be in place for a successful readmissions program.
