Health Services Advisory Group (HSAG) working with CALTCM’s Quality & Safety Committee, has launched a new program to help nursing homes prepare for the CMS Value-Based Purchasing (SNF VBP) readmission penalties, which will go into effect October 1, 2018, although the data are already being gathered for benchmarking. Join this nine-month journey (January – September 2018) to implement prevention strategies that should be in place for a successful readmissions program.
