Title: SNF 2.0 – INTERACT-ing Beyond 6 Months

Summary:  Six-month results for “unengaged” SNFs were poor in the 2011 INTERACT study. Reasons for lack of success included loss of champions, administrative turnover, and problems reporting data. We developed SNF 2.0, a mentorship+incentive program, to improve successful INTERACT engagement. SNF 2.0 was piloted at two SNFs with INTERACT engagement challenges. Last year, we reported data at 7-9 months post-mentor introduction (PMI), showing a >34% reduction in 30-day SNF-to-Hospital readmissions (30R) compared to 2012 facility-specific data and reaching our “very good”-“excellent” patient satisfaction goal (PSG) of ≥90%. Here, we report our data 18 months out.

We partnered with two SNFs that had previously implemented INTERACT providing one-on-one mentorship and group training to licensed nurses and nurse aides. Mentorship consisted of shadowing individual staff and using teachable moments to help integrate INTERACT philosophy and tools into their daily workflow.  A “gamified” dashboard showing use of Stop and Watches and SBARs, readmission rates, and patient satisfaction were updated weekly and displayed prominently to keep staff engaged.Mentorship+incentives may help SNFs find success with INTERACT despite early turnover in staff, but prolonged periods of turnover may reverse gains.
