Parag Agnihotri, MD, CMD 

CALTCM BOD Member

Dr. Parag Agnihotri currently serves as the Medical Director for Continuum of Care for Sharp Rees-Stealy Medical Group, San Diego. His work focuses on reforming the delivery of health care by building multidisciplinary teams to implement the ‘three part aim’ for improving the health of the population. He is a California HealthCare Foundation Fellow. The Center for Medicare and Medicaid Innovation (CMMI) has designated him as an Innovation Advisor.

Dr. Agnihotri completed his residency and chief residency in Internal Medicine and subsequent fellowship in Geriatric Medicine from University of Illinois, Chicago. He is board certified in Internal Medicine and Geriatric Medicine, and has served on CALTCM’s Board of Directors since 2009.

Interview with Parag Agnihotri:

CALTCM:  Brief background of your training and practice setting and years in practice.

Dr. Agnihotri:  I am Board-Certified in Internal Medicine and Geriatrics with CMD from AMDA. I completed IM Residency training, Chief Residency and then Geriatric Fellowship from University of Illinois at Chicago’s training program.  Subsequently I completed a California HealthCare Foundation leadership fellowship and was also one of the CMMI’s innovation advisors.  I have been in practice now for 14 years. I enjoy practicing in Post -Acute & Long-Term Care and Home-based care program. 

CALTCM:  What are you passionate about in long term care?  How are you pursuing your passion?

Dr. Agnihotri: Post Acute care fascinates me as I get to practice medicine in a comprehensive manner and with a team of dedicated professionals. The principles of ‘Geriatric Medicine’ can be practiced in a relatively practical way in the PA/LTC setting. The work is fulfilling as one gets to see a variety of medical cases, be the provider of care for patients and their families, and be part of the health care administration that continues to grow. 

CALTCM:  What advice would you give to a new graduate contemplating a career in long term care?

Dr. Agnihotri:  There are many opportunities in Post Acute care. One gets to practice good geriatric medicine and can also pursue a career as health care leader for a team of dedicated professionals at the facility or collaborators. The career is financially rewarding, has flexible work hours and you will be always in demand for years to come. More and more large healthcare systems are embracing PA/LTC as one important component of their health care delivery. Being part of a passionate  and skilled team who is working on improving health care is one of the joys of this career. 

CALTCM: What additional training do you wish you had that may help with your current practice?

Dr. Agnihotri:  Healthcare is rapidly changing, and with it comes a host of new health care policies and regulations. A training to understand the policies better so as they can be implemented effectively would be helpful. 

CALTCM:  How has CALTCM impacted your practice?

Dr. Agnihotri: CALTCM has tirelessly worked on promoting statewide efforts to advocate quality patient healthcare, provide long-term care education, and influence policy. Our teams and I have benefited from the ongoing education opportunities and participation in Quality Improvement projects. These activities have directly benefited our patients at the facilities. We have participated in sharing best practices via the annual poster sessions. CALTCM has worked on advocating for or against various state assembly bills that affect the health care policies for  PA/LTC. Sometime this work is not readily visible to us as members. We are beneficiaries of the statewide advocacy on our behalf and we are thankful to CALTCM.
