CMS has announced that by 2016, 30% of Medicare reimbursements will be through coordinated care organizations and by 2018, this will be 50%.  In addition, providers not in an Accountable Care Organization (ACO) or bundled payment system would receive 90% of their payment through quality measures.

While this proposed plan may take longer than CMS predicts, it’s clear that CMS is promoting the triple aim of better patient care at lesser cost with better outcomes through integrated and care coordinated approaches.

At CALTCM, we have designed our annual meeting to promote improvements that support the triple aim.  The day of “passive” physician care with care assessments and care plan development deferred to facility nurses and rehab teams is being challenged by physicians focused on Post-Acute and Long-Term Care (PA/LTC).
