We are pleased to invite you to attend the CALTCM 2015 Annual Meeting on April 24 & 25 at the Omni Hotel Los Angeles at California Plaza.   This year’s annual meeting will shine the spotlight on “2015 Hot Topics”, “Care Coordination”, and “Patient Safety”.  Our course will be preceded by a half-day pre-conference seminar on a proven method for team building in the care of dementia patients across the LTC spectrum.

Our “2015 Hot topics” session will focus on a series of concise updates on clinical topics pursuing our goal of fostering clinical excellence.  Our understanding of Care Transitions has matured, realizing that “Care Coordination” is what our patients really need at every point in the transitions through our healthcare system.  Our “Patient Safety” workshop will not only feature presentations on high-risk areas, but also end with a Mock Trial representing real world issues encountered in an elder abuse lawsuit.  Mindful that this is the year QAPI (Quality Assurance Performance Improvement) goes live, you will have the opportunity at our conference to develop a PIP (Performance Improvement Project) for your facility.  In addition, we anticipate another robust POSTER Session showcasing your PIPS of the past year.
