Don’t fall back! Come join us for our 2nd Annual Fall CALTCM conference at UC Davis in a new state-of-the-art teaching center that takes the teacher off the podium and puts learners in charge of applying the material to their work-life right away. Phase II of the new CMS regulations are here – are you ready?

Get ready for brief lectures and interactive exercises that will keep you off your phone through our “Propeller” learning stations. Each station seats 6-7 people with a screen for viewing the teaching material, cases, and questions. The screen can then be used for your team’s response when a computer is connected to it. In this format, you will not only be a student, but also a teacher, active learner, and team builder. Each instructor will provide a summary with links to resources and tools for your expanded future use.

This learning experience will cover common problem areas in the PA/LTC (Post-Acute and Long-Term Care) setting paired with a curriculum that is fast-paced and practical. You can expect to learn about pain assessment and management, comprehensive depression care, individualized diabetes care, minimizing MDROs (Multi-Drug-Resistant Organisms), advances in heart failure and anticoagulation, and of course an update on regulatory and public policy issues that impact the fast-changing PA/LTC environment and the patients you serve.

This meeting has elements appropriate for all members of the PA/LTC team. As a Saturday morning meeting, your facility “Survey Window” shouldn’t be a barrier to attendance. This is a great time of year for performance improvement. Target Audience

Physicians, advanced practice nurses, geriatric psychiatrists, pharmacists, nurses (DON/RN/LVN/DSD), administrators, physician assistants, rehabilitation special-ists, social services designees, and other members of the interdisciplinary team who care for patients/residents in the PA/LTC setting. 

Learning Objectives:

At the end of the course, participants should be able to:

  1. Describe the components of an effective and compliant QAPI program
  2. Identify 3 specific concerns with the new guidelines for pain management in the PA/ LTC setting
  3. Recognize the difference between partial and full remission of major depressive disorder
  4. Develop individualized glycemic care plans for their patients with diabetes
  5. Determine changes in public policy and regulations that require adjustments to facility policies and strategic plans.
  6. Identify changes in your facility’s infection prevention program to reduce the prevalence of MDROs.
  7. Create care processes to support the safe use of advances in heart failure and anticoagulation.
