Program Introduction

The 2016 CALTCM Fall Conference, The Best of Quality Through Best Practices, continues our traditional focus on person-centered care in the long-term care setting. This event will challenge attendees to find ways to integrate the latest knowledge into their post-acute and long-term care (PA/LTC) practices. This meeting is designed for practical training of all members of the interdisciplinary team, focusing on areas where PA/LTC is presently aimed to improve quality. 

The 2016 Fall Conference highlights the top scoring topics from the 42nd Annual Meeting followed by a question and answer session after each presentation. Featured topics: Reducing Antipsychotic Use, Diagnosing Infections, “De-Prescribing”, POLST, Aid in Dying, GeriatricPain.org, Sex in the Nursing Home, and Bundled Payments. Target Audience

Physicians, advanced practice nurses, geriatric psychiatrists, pharmacists, nurses (DON/RN/LVN/DSD), administrators, physician assistants, rehabilitation special-ists, social services designees, and other members of the interdisciplinary team who care for patients/residents in the PA/LTC setting. 


Learning Objectives

By participation in the 2016 Fall Conference, participants will have the ability to: 

  1. Demonstrate Antibiotic Stewardship for UTI’s – cite practical steps facilities can take to comply with new CMS expectations;
  2. Identify best practices for achieving safe medication care plans;
  3. Describe accommodating residents sexual desires in ways that protect vulnerable patients yet honors patients autonomy;
  4. Summarize updates on changes from volume based to value based payments for facilities and providers;
  5. Describe the care of persistent non-cancer pain in view of new CDC pain recommendations;
  6. Prepare facilities for requests from residents for “Aid in Dying”.
