CALTCM Best Practice Implementation Award
Call for Submissions

 

As a part of CALTCM’s mission to improve quality patient care across the long-term care continuum, we are looking for providers (individuals or institutions) that demonstrate successful implementation of best practices with a plan to ensure sustainability. Programs include those shown to yield positive care outcomes. Eligible Best Practices include innovations in care processes focused on enhancing and improving the care of patients with multiple chronic conditions and end-of-life care needs.  All practices must be evidence based.

Submissions will demonstrate best practices with a proven track record that can be spread throughout the long term care community.

Submitting your best practice:

  • Step 1 - Identify the program team lead. This is someone who can speak to the organization’s best practices related to improving the care of patients with multiple chronic conditions and end-of-life care. This may be you, but if not, please share your designee’s contact information with us.
  • Step 2 - Submissions will include a brief paragraph description of the best practice, which will include a detailed implementation plan.  (See Examples Of Best Practices below.)

Submission Deadline EXTENDED: Monday December 5, 2016

  • A CALTCM selection committee will choose the top five (5) submissions that have demonstrated potential for successful implementation to present their Best Practice at the CALTCM 2017 Annual Meeting Pre-Conference entitled Best Practice Summit 2017.
  • The top three best practice presentations showing the best implementation and best sustainability plan will be recognized at the 2017 CALTCM Annual Meeting Awards ceremony on Saturday, April 29, 2017.
  • 1st Place- $1,500  
  • 2nd Place- $750  
  • 3rd Place– 1 year free CALTCM Membership

  1. Methods for prevention and management of injurious fall;
  2. Interventions or methods addressing prevention and management of urinary tract infection;
  3. Interventions or methods addressing recognition and management of moderate to severe pain;
  4. Interventions or methods addressing prevention and management of low-risk residents with pressure ulcers;
  5. Interventions or methods addressing recognition and management of urinary incontinence;
  6. Interventions or methods addressing recognition and management weight loss and under-nutrition;
  7. Methods addressing recognition and management of obesity;
  8. Methods identifying and addressing depressive symptoms;
  9. Management of residents with behavioral disturbance that disrupt care of other residents;
  10. Methods addressing newly admitted residents who receive antipsychotic medications;
  11. Methods for early detection and management of resident change in health status;
  12. Care processes to reduce hospital admissions and re-admissions for ambulatory sensitive care conditions;
  13. Interventions or methods to obtain goals of care and execution of advance care directive;
  14. Methods addressing drug reconciliation and prevention of adverse drug reaction;
  15. Methods to ensure appropriate use of anti-infective drugs (antibiotics);
  16. Programs to ensure patient safety through effective infection control;
  17. Care processes for management of Diabetes Mellitus;
  18. Care processes for management of congestive heart failure;
  19. Care processes for management of chronic kidney disease;
  20. Care processes for management of end stage renal disease (ESRD).
