American Society of Consultant Pharmacists The American Society of Consultant Pharmacists empowers pharmacists to enhance quality of care for all older persons through the appropriate use of medication and the promotion of healthy aging. ASCP’s 7,000+ members manage and improve drug therapy and improve the quality of life of geriatric patients and other individuals residing in a variety of environments, including nursing facilities, subacute care and assisted living facilities, psychiatric hospitals, hospice programs, and home and community-based care. In addition, 4,500+ pharmacy student members of ASCP demonstrate their identification with and support of ASCP's mission and vision to serve seniors wherever they reside. California Coalition for Compassionate Care Supported by a grant from the California HealthCare Foundation, the Coalition for Compassionate Care of California leads the California POLST Project. POLST (Physician Orders for Life-Sustaining Treatment) is a form that states what kind of medical treatment patients want toward the end of their lives. Advancing Excellence in America's Comprised of long term care providers, medical professionals, consumers, employees, and state and federal agencies, AE is the largest and first coalition of its kind to measure quality by setting clinical and organizational goals for nursing homes. The coalition stimulates quality improvements by providing nursing homes with free, current and practical evidence-based resources, empowering residents and their families with education, and helping participants reach their targets.
